Manipal Hospital Goa has performed what it describes as the state’s first covered self-expandable metallic stent placement to treat a complex case of tracheal stenosis accompanied by a tracheoesophageal fistula, marking a development in the management of critical airway conditions locally.
The patient had been suffering from severe breathing difficulty caused by a critical narrowing of the airway. The condition was attributed to compression from an underlying esophageal tumour, which not only obstructed airflow but also led to the formation of a tracheoesophageal fistula—an abnormal connection between the trachea and the esophagus. This complication made the case life-threatening and required urgent medical intervention.
The procedure was carried out by the Department of Pulmonology, led by Dr John Muchahary, with Dr Ashwini Colaco and Dr Shekhar Salkar, and supported by anesthesiologist Dr Yogesh Gaude. The team undertook coordinated planning to address both the airway obstruction and the associated fistula.
During the intervention, a covered self-expandable metallic stent was deployed within the trachea. The device helped reopen the narrowed airway, restoring patency, while also sealing the fistula to prevent further complications. Doctors reported immediate improvement in the patient’s breathing following the procedure.
Tracheal stenosis involves narrowing of the airway and can significantly impair breathing and oxygen supply. When combined with a tracheoesophageal fistula, the risks increase, as food and liquids may enter the airway, leading to aspiration and severe respiratory complications. In such cases, minimally invasive airway stenting is used to provide structural support and isolate abnormal connections.
Commenting on the case, Dr John Muchahary said, “This was a highly complex and high-risk case where timely intervention was crucial. The successful placement of a covered self-expandable metallic stent not only restored the patient’s airway but also effectively sealed the fistula. Such advanced procedures significantly improve patient outcomes and reinforce our commitment to delivering cutting-edge pulmonary care within Goa.”
